Chapman University has access to five different software packages for surveys and forms.
Qualtrics
Chapman has a university-wide subscription with Qualtrics, an online survey software. Available to Chapman University faculty, staff, and administrators for academic, administrative, and research-related purposes, it is a sophisticated tool that is easy to use for creating and deploying surveys and analyzing survey responses.
Key features include but are not limited to survey builder, skip logic, an automated summary of survey progress/response rate, language translation tools, collaboration, centralized management of survey communications, reporting tools, and exporting capabilities—including CSV, XML, SPSS, etc.
Who can use Qualtrics?
- All Chapman faculty and staff
- Chapman students in the following groups
- Student employees in the course of their employment
- Graduate student researchers for academic research projects
- Undergraduate student researchers with approval from their faculty sponsor/mentor

How do I access Qualtrics?
Formstack
Chapman administration and staff have access to Formstack, an online forms software. It starts with a simple-to-learn and simple-to-use drag-and-drop form interface. Forms can be used via a link or easily embedded on any Chapman webpage.
Advanced builders can use Formstack to create simple workflows, actions triggered in the form that allows users to automate routine processes and perform actions.
Who can use Formstack?
- Chapman staff, faculty, and student employees with Formstack accounts can create surveys/forms
- Anyone can fill out a survey/form
- Workflow participants need a “participant” Formstack account to approve/deny requests
What are the primary uses of Formstack?
- Surveys and forms to be embedded on Chapman-hosted web pages
- Simple surveys and forms that need to be Chapman-branded
- Administrative forms with basic workflows
How do I access Formstack?
- Login to Formstack
- Select “Log in with Azure SAML SOO”
- If the above button is not visible, enter your Chapman email address and select “Next”
Kuali Build
Chapman has a campus-wide subscription to Kauli Build, allowing us to build advanced forms and workflows. One of Build’s strong points is its ability to easily use external data sources as options within forms and workflows without making the user enter data already collected.
Who can use Build?
- All Chapman faculty, staff, and students will be able to fill out forms and participate in workflows as appropriate
- For now, forms and workflows will be created by IS&T. If you need a complex workflow, please submit a Service Desk Request

Microsoft Forms
Microsoft Forms is included in Chapman’s Microsoft license, so all students, faculty, and staff. It allows for simple surveys and quizzes. The data can be exported to Excel.
Who can use Microsoft Forms?
- All Chapman faculty, staff, and students have access via their Microsoft Office 365 license
What are the primary uses of Microsoft Forms?
- Simple, quick, non-webpage embedded forms or quizzes
- No PII (personally identifiable information) should be included in the form
- PII includes name, address, SSN, phone number, passport number, driver’s license number, and taxpayer ID.
How do I access Microsoft Forms?
Google Forms
Google Forms is included as part of the Google Suite and is available to all Chapman faculty, staff, and students. Features include but are not limited to, shuffling of questions for randomized order, limiting responses to once per person, shorter URLs, custom themes, automatically generating answer suggestions when creating forms, and an "Upload file" option for users answering questions that require them to share content or files from their computer or Google Drive.
