For students entering Chapman in the fall, all immunization records (outlined in the table above) must be uploaded to the Medical Portal by August 1.

In order to be enrolled at Chapman in the fall semester, you must upload your documents before this date. If you fail to upload your records by this date, you may be unable to attend Chapman. You may also need to pay late fees if you submit your records late.

Immunization exemptions

For students entering Chapman in the fall, the deadline to request an immunization exemption for any reason (non-religious or religious) is August 1. We strongly encourage you to request any immunization exemptions well in advance of the deadline to provide enough time for review.

Requesting an immunization exemption for non-religious reasons

To request a medical immunization exemption for non-religious reasons, you must submit a letter from an appropriate MD or DO. This letter should:

  • Be written by an MD or DO who is licensed at the time of writing.
  • Indicate the reason for your exemption request.
  • Be dated within the last one year.

Frequently asked questions

Toggle Section

What is a titer test and why would I submit one?

If you do not have access to records, you may be able to submit a titer test. A titer is a laboratory test, ordered by a medical provider, that measures the presence and amount of antibodies in the blood. A titer may be used to prove immunity to disease. This option should only be used by those who no longer have access to immunization records. Titers may only be accepted for MMR, Varicella and Hepatitis B.

What if I need an extension on the deadline?

If you need an extension to the deadline, you can email studenthealth@chapman.edu. In your email, request an extension to the August 1 deadline. Your extension will be valid until 14 days after you are covered by Student Health Insurance. Before emailing in your request, please upload any immunization records you do have so that staff are fully informed of your immunization status when reviewing your extension request. Extensions will be made on a case-by-case basis.

Do international students have more time to submit their information?

Yes. International students will have an additional 30 days to submit any outstanding requirements (September 1), as we understand some of the vaccines are not available in every country. However, please upload all records you do have by the August 1 deadline.
