Deadline for submitting immunization records
For students entering Chapman in the fall, all immunization records (outlined in the table above) must be uploaded to the Medical Portal by August 1.
In order to be enrolled at Chapman in the fall semester, you must upload your documents
before this date. If you fail to upload your records by this date, you may be unable
to attend Chapman. You may also need to pay late fees if you submit your records late.
Immunization exemptions
For students entering Chapman in the fall, the deadline to request an immunization exemption for any reason (non-religious or religious) is August 1. We strongly encourage you to request any immunization exemptions well in advance of the deadline to provide enough time for review.
Requesting an immunization exemption for non-religious reasons
To request a medical immunization exemption for non-religious reasons, you must submit a letter from an appropriate MD or DO. This letter should:
- Be written by an MD or DO who is licensed at the time of writing.
- Indicate the reason for your exemption request.
- Be dated within the last one year.
Requesting an immunization exemption for religious reasons
To request a medical immunization exemption for religious reasons, you must complete the Religious Exception from Vaccine Requirement Request Form.
