Cayuse IACUC (Animal Oversight) Protocol Processing Links:
- The URL for the production site is: https://chapman.esirius.cayuse.com/esirius3g. This is where real protocols are to be completed.
- The URL for the training site is: https://chapman.ntm-stg.cayuse.com/esirius3g. Note the "Non-Production" banner in red across the top. Protocols written in training cannot be moved to production - content can be cut-and-pasted, however.
- Email the IACUC office to be authenticated or registered to use Cayuse IACUC. See the related tab on User Authentication.

To begin the IACUC protocol preparation process, please download and complete the following forms as required.
Safety
- Risk Assessment Safety Questionnaire ( RASQ) - version 4/21/2021, for use by "casual" visitors to animal facilities (observing for brief times and not coming into contact with animals) at the time of entry
- Laboratory Animal Occupational Health Questionnaire (LAOHQ) - version 1/2020, for use by persons (students, faculty, research staff, etc.) who
have direct contact with animals
