International Supplement Form

» Completing the Graduate International Supplement Form

All graduate international students are required to submit an International Supplement Form as part of their application. This form will be available in your Application Status Page once you are admitted. If you applied using a CAS application (M.S. or Ph.D. in Pharmaceutical Sciences, M.S. in Communication Sciences and Disorders, or Doctor of Physical Therapy), please use this form

The International Supplement Form provides Chapman University with information about your country of citizenship, current visa status, passport and, for students who intend to hold an F-1 student visa, funding for your education.

You will be asked to submit the following documents as part of your International Supplement Form:

  • Photocopy of passport data and signature page
  • Evidence of available funds (see dropdown menus below)

You will also be asked about the type of visa you currently hold and type you intend to hold at Chapman.

Give yourself enough time to gather all the documents needed for your International Supplement Form so you can fill out all the required fields.

Evidence of Available Funds

Applicants must submit evidence for each type of funding that applies to their situation. Acceptable forms of evidence documentation are listed under each category.

Your total amount of funding must be equal to or greater than Chapman’s cost of attendance for international graduate students.

Personal Funds

Submit one of the following documents as evidence of personal funds:

  • Bank/financial statement in your name that shows your account balance. This can include multiple accounts. Your bank/financial statement must:
    • Indicate the financial institution’s name
    • Indicate the account holder’s name
    • Clearly show the account balance
    • Be dated within the past 6 months (older statements will not be accepted)
    • Be translated into English, including names
    • Clearly indicate the type of currency
    • Show liquid assets that can be converted to cash

Family Funds

Evidence of funds from family must include:

  • Your family’s bank/financial statements (see criteria under the Personal Funds section above)
  • Letter(s) indicating the relationship between you and the sponsor(s), as well as the amount of yearly support available 
As an alternative to a letter from your sponsor, you may submit the Financial Sponsor Information Form along with the sponsor’s bank/financial statements.

Funding From Other Sources

Funding from other sources may include an employer or organization. Proof of this funding must be written on official business letterhead and should include a signature.  The amount of yearly funding available from this source should also be included.

Salary statements are not accepted.

Loan Certificates/Letters

Loan certificates/letters must include your name and the amount of the approved loan on official letterhead. Conditional loan approvals may be accepted only if the condition is the receipt of your I-20 or DS-2019.

Loan applications are not accepted as evidence of funding.

The following documents are not accepted as evidence of funds:

  • Documents older than 6 months
  • Income or salary statements
  • Funds that are not immediately accessible
  • Tax return forms or documents
  • Assets that are not in liquid form, such as a house, car, etc.
  • Life insurance policy statements
  • Pension funds
  • Loan applications

Receiving Your I-20/DS-2019 Form

Once you have been admitted to Chapman and have accepted your admission by submitting your enrollment deposit, your I-20/DS-2019 form will be issued by our International Student Services office. For additional information, please visit our Immigration Documents page.
