International Supplement Form

» International Supplement Form

As an international graduate student at Chapman, you are required to submit an International Supplement Form after you have been admitted.

What is the International Supplement Form?

The International Supplement Form is a form that you must submit to begin the immigration process for studying in the United States.

The International Supplement Form lets us know:

  • Your country of citizenship.
  • Your current visa status.
  • Your sponsorship and financial information (if you will need an F-1/J-1 visa).

When do I submit my International Supplement Form?

You will submit your International Supplement Form after you have been admitted to Chapman.

How do I submit my International Supplement Form?

After you have been admitted to Chapman, find your International Supplement Form in your Application Status Page. There, you can fill out the form and submit it.

If you applied to the program using a CAS application and do not have a login to the Application Status Page, then use this version of the International Supplement Form instead. Then submit the completed form and all additional documents to Graduate Admission Counselor, Shannon Morris Alvarez at shmorris@chapman.edu

What documents do I need to submit with the form?

You will be asked to submit the following documents as part of your International Supplement Form:

  • Photocopy of passport data and signature page.
  • Evidence of available funds. These may be personal funds, family funds, funding from other sources, or loan certificates/letters. Your available funds must be equal to or greater than Chapman’s cost of attendance for graduate students. For immigration purposes, you are required to show one year's worth of tuition and costs. You can find the tuition and additional costs of your program on our International Tuition and Costs page

Make sure you don’t forget these documents. We can’t accept your International Supplement Form without them. Also, please ensure that your financial documents are in English; if they are not available in English, you will need to provide an official word-by-word, line-by-line translation along with the original language.

You will also be asked about the type of visa you currently hold (if any) and the type you plan to hold at Chapman.

How much does Chapman cost?

Chapman's cost varies depending on the program. Each program has its own tuition, plus additional costs. You can view tuition costs per credit for each individual program on our Student Business Services website

*For your I-20 cost of attendance, you are required to show one year's worth of tuition and additional costs.

What counts as evidence of available funds?

There are four types of funding that you can provide as evidence of your available funds. The type you provide depends on your individual situation.
