Missing Application Items

»How to Submit Missing Application Items

See below for a list of items that might be missing from your application to Chapman University and directions on how to submit them. Note that it may take 1-2 weeks for us to process materials after we have received them.

  • Audition Callback: Select applicants to our performing arts majors will get invited to a virtual or in-person callback audition. If you are missing your audition callback, you should check your email with the registration information.
  • Creative Supplement: Select creative majors require a Creative Supplement. You will submit your Creative Supplement in your Application Status Page. You can find more details on the Creative Supplement here.
  • Early Decision Agreement: Those applying Early Decision must submit an Early Decision agreement. This is located within the Common Application.
  • English-language Proficiency Exam: International students may have to submit an English-language proficiency. You can learn more about acceptable tests and minimum scores on our International Admission page. Also note:
    • There is also an English Proficiency Waiver Request in your Application Status Page. You can submit this if your are in an English-language-based curriculum (American-patterned, Cambridge or the IB).
    • If you have not taken your exam and need to submit your test soon, we recommend the Duolingo English Test. The DET can be taken anytime and anywhere. Results will be received by Chapman within 48 hours. Note that our minimum DET requirement is 120.
  • High School Transcripts (unofficial): If requested, we can accept unofficial transcripts to review first-year applications (though first-year students must submit official transcripts after accepting admission to Chapman). Students can upload digital copies of their transcript in their Application Status Page.
  • High School Transcripts (official): Transcripts are not considered official if they are sent to us from the student. For a transcript to be considered official, it must either be:
    • Submitted electronically as a PDF to Chapman directly from the issuing institution. 
    • Mailed to Chapman in its original sealed envelope. See below for directions to send them to the Office of Admission.
  • International Supplement Form: The International Supplement Form is required for international applicants. It is located in the Application Status Page. You can find more details on the International Supplement Form here.
  • Mid-term Report: The Mid-term Report is for transfer applicants. You can download the Mid-Term Report form here. Please print and fill it out approximately halfway through your current term (grades can be self-reported and the report does not need to be signed). Once complete, scan and email it transfers@chapman.edu. Do not wait until receiving final grades. For more information, view our Transfer Reports page.
  • Mid-year Transcript: Your Mid-Year Transcript should be requested after your fall semester or trimester grades have been posted. Please request it be submitted as soon as grades are available. See below for directions to send them to the Office of Admission.
  • School Report: The School Report form is submitted by your high school counselor or school official. See below for directions to send them to the Office of Admission.
  • Teacher Evaluation (Letter of Recommendation): You can request your Teacher Evaluation (also known as Letter of Recommendation) through the Recommender section on the Common App. You or your recommender can also send the Teacher Evaluation to us at admission@chapman.edu.
  • Test Scores (Unofficial): If you indicated you want to apply with test scores, you will be able to self-report your scores. Chapman will superscore your SAT or ACT scores.
    • If you are looking to send official test scores, Chapman’s Testing Code for the SAT is 4047; for the ACT it is 0210.
  • Test Scores (Official): If you accept admission to Chapman and applied with unofficial test scores, you will need to submit an official copy of your scores. To do so:


Sending official school reports and transcripts to Chapman

Transcripts that are sent to us from the student are not considered official. For a transcript to be considered official, it must either:

  • Be submitted electronically as a PDF to Chapman directly from the issuing institution.  The Admission office email address for transcript delivery is admtranscript@chapman.edu.
  • Remain in its original sealed envelope when mailing documents to: Chapman University, Attn: Office of Admission, One University Drive, Orange, CA 92866
