A visual representation of the request process which has been described below.

Voluntary Flexible Work Arrangements (VFWA) are discretionary and subject to periodic review. A VFWA request can only be initiated by the employee. Once the request has been submitted using the online form, it will be automatically routed to the employee's supervisor. The supervisor will review the request with primary focus on the organizational impact and secondary focus on the employee's eligibility and suitability.

If the supervisor indicates support for the request, and the request does not involve the employee to work out of the state of California, the request will then be routed to the corresponding Dean/Department Head. If the Dean/Department Head indicates support for the request, the request will then be routed to the relevant Senior Staff member for their decision. If the Senior Staff member approves the request, an agreement is generated and emailed to the employee and the supervisor to sign.

If the supervisor indicates support for the request, and the request does involve the employee to work out of the state of California, the request will then be routed to the Office of Human Resources (HR) for review. HR will work closely with the relevant Dean/Department Head, Senior Staff member, and the Office of Legal Affairs to assess the feasibility of the out-of-state voluntary flexible work arrangement request. Please note that review of out-of-state requests take considerable time. If authorized, HR will forward the request to the corresponding Dean/Department Head. If the Dean/Department Head indicates support for the request, the request will then be routed to the relevant Senior Staff member for their decision. If the Senior Staff member approves the request, an agreement is generated and emailed to the employee and the supervisor to sign.

Once the agreement form has been signed by the employee, the supervisor, and the Senior Staff member, a PDF copy will be emailed to the employee, the supervisor, and the Office of Human Resources for recordkeeping.

The Senior Staff leader's decision is final.
