Request Form
A request for voluntary flexible work arrangements (VFWA) can only be submitted by an employee to place a request for themselves. We strongly encourage you to do all of the following prior to filling out and submitting your request form.
- Read the policy in full
- Review the Overview of Procedures section
- Review the Resources for Staff section
- Discuss your intended proposal with your supervisor
Cancellation Form
A voluntary flexible work arrangement (VFWA) agreement may be revoked by either party at any time with a two week courtesy notice where feasible. The cancellation form must be submitted by the initiating party.
