I, undersigned, hereby grant permission to Chapman University and its agents and licensees (collectively “University”) to tape, video, film, record and/or photograph me, including my name, image, likeness and performance, in the project or works produced on the date and location listed below (the “Works”). I hereby grant permission to University to audio record my voice, conversation, quotes, paraphrases and sounds in the Works. I agree that there will be no residual, royalty, payment or other fee due me, or any third-party, in connection with my appearance or performance in the Works, its publication, broadcast, podcast, rebroadcast, online, streaming, licensing, sublicensing, sale or assignment.
I hereby waive any moral, false light, privacy, misuse, and publicity rights that I may have in connection with the Works and any uses thereof. I agree that University and its successors and assigns shall be the exclusive owner of all copyrights and title in and to the Works. I hereby irrevocably grant permission in perpetuity to University, its successor or assigns, to use my name, image, likeness, voice and recording in the Works, forever and throughout the universe, in any and all media, websites and platforms, now known or hereafter discovered, along with any incidental uses in connection with the merchandising, marketing and promotion of the Works.
I hereby grant permission and irrevocably assign to University, its successor or assigns, the world- wide rights in perpetuity, to use, own, sublicense, transfer and copy the Works or any excerpts or derivatives thereof, including my name, image, voice, likeness and any related or derivative versions of my performance or content in the Works, including translation, publication, foreign rights, serialization, syndication, photocopying, abridgement, adaptation, reprint, dramatization, and electronic recording and reproduction of any sort, in print, online, other content, electronic, digital, audio, video and any other media forever throughout the universe for any purpose, educational, commercial or otherwise.
I hereby agree to release and hold harmless the University and its agents, employees and assigns, including any firm publishing and/or distributing the finished product in whole or in part, from and against any claims, damages or liability arising from or related to the use of the Works, including but not limited to libel, false light, invasion of privacy, rights of publicity, copyright infringement, misuse, distortion, blurring, alteration, optical illusion or use in composite form, either intentionally or otherwise, that may occur in taking, processing, reduction or production of the finished Works, its publication or distribution.
I am eighteen (18) years of age or older, and I am competent to contract in my own name. I have read this release before signing below, and I fully understand the contents, meaning and impact of this release.
