»Microbiology 317L At-Home-Lab Form

 

We are excited to send you an at-home-micro lab kit for the fall 2020 semester. In order to participate in these labs, you must first read through all safety information and agree (with signature) to follow all safety precautions while at home. All kits will be mailed, so please provide the address that you would prefer the kit be shipped to at the bottom of this form.

  1. I agree to follow all safety measures provided in my at-home lab safety sheet as described below.
  2. I agree to report any contamination or damage to my at-home lab kit promptly.
  3. I agree to follow all proper disposal instructions (these will be included with each individual lab).

Safety Rules and Procedures

  1. Notify your lab instructor immediately of any accident or injury that occurs in the lab. For Emergency Response to any injury or accident seek medical attention immediately.
  2. Wear the proper laboratory attire, i.e. personal protective equipment (PPE), consistent with the chemicals and other materials, equipment and procedures performed in the lab. You are required to wear the designated PPE throughout the lab activity, appropriate gloves, and closed toes shoes. Tie back hair to keep it away from flames and chemicals. It is your responsibility to solicit the PPE guideline from your instructor and to comply fully.
  3. Read and attend to all warnings given by the instructor.
  4. Read chemical labels and lab Standard Operating Procedures very carefully. Serious injury or death can result from mixing the wrong chemicals. Review SDS’s for any chemical that is unfamiliar to you and consult with instructor.
  5. Never pipet by mouth; use pipet pumps. Never taste a chemical. Check odors only if instructed to do so by gently wafting some of the vapor towards your nose with your hand.
  6. Wash skin immediately and thoroughly if contaminated by chemicals or microorganisms.
  7. Be aware of electrical sources. Do not allow any liquid to come into contact with electrical connectors. Do not attempt to disconnect any equipment that crackles, snaps or smokes.
  8. Always dispose of materials in properly. Gloves, swabs, bacteria plates, body fluids and other biological material must be disposed of in an autoclavable biohazard bag. Waste material will be mixed with 10% or more bleach before disposal. When in doubt, ask your instructor where to dispose of material.
  9. Always wash your hands at the beginning and end of lab experiments.
  10. Always be aware of your lab surroundings so as not to cause harm to those around you.
  11. Clean your work surface when you are done with your protocols. Leaving your work station dirty or unorganized may leave contaminants exposed that could harm incoming persons.
  12. No drinking or eating during lab experiments. This can result in contamination of media or your food.
  13. I agree to advise my instructor of any allergies or medical problems that could impact my health and safety.
