What is Dean's Clearance?

A Dean's Clearance, also known as Disciplinary Clearance or Dean's Certification, verifies your conduct record. It is frequently required by graduate schools, study abroad programs, state bar associations, government agencies, and private organizations when applying for admission or employment.  

Our procedures may only allow us to send these documents directly to the requesting institution. 

Protecting Privacy

The Family Educational Rights and Privacy Act (FERPA) affords students certain rights with respect to their education records, including the right to inspect their education records, request an amendment of the records that the student believes are inaccurate, and the right to control disclosures of their records except to the extent that FERPA authorizes disclosure without consent. This means: 

  • Third parties seeking information about a student's disciplinary record must have the student's written authorization for us to disclose the student's disciplinary history. 
  • Students and former students must provide identifying information, such as date of birth or student identification number. 
  • All forms must also be signed and dated.

There are several types of Dean's Clearance requests. Guidelines for submitting each request type can be found below. If the clearance you need is not listed below, you can email deansclearance@chapman.edu.

Study Abroad Clearance

Independent Study Abroad Programs
Students choosing to study abroad independently, please refer to the instructions from the Office of the Registrar's Articulation page to learn more about student responsibility, status, transfer credits, financial aid, and support from Chapman. 

All independent study abroad disciplinary clerance forms are submitted directly to the study abroad program direclty. If you are provided with a discipinary clearance form, please complete the following steps: 

Step 1
Complete the student portion of the form, including the FERPA Release Authorization and date. Please make sure all required areas under the student section are completed. We cannot complete the form without your signature. 

Step 2
After you have completed the required student section, email it to deansclearance@chapman.edu from your Chapman email, also providing an email address for the study abroad program. 

Step 3
We will send the form directly to the study abroad program. You will receive a confirmatoin email when it has been sent.

Transfer Clearance

When students apply to transfer, some colleges or universities may require them to complete and submit forms. One such form is often called a Disciplinary Clearance Form, which reports the student's conduct during their time at Chapman. 

The name of the form varies from campus to campus. Examples include:

Statement of Good Disciplinary Standing

It is the applicant's responsibility to ascertain which forms are necessary for the institution's application process. To ensure timely submission and avoid delays or rejection, applicants are strongly encouraged to contact the Office of Student Conduct well before the application deadline. 


Step 1

Step 2
After you have completed the required student section, email it to deansclearance@chapman.edu from your Chapman email. If you are unable to access your Chapman email, we may require you to answer additional questions to verify your identity.  

In the email, please attach or include the following: 

Clearance Form, completed as shown above
Full Name & Student ID Number
List of Universities & their email addresses

Step 3
We will send the form directly to the requested Universities. You will receive a confirmation email when these have been sent.

Institutional, Governmental, and Organizational Requests

Institutional, Governmental, and Organizational Requests apply to the following: Law Schools, Medical Schools, State Bar Associations, Government Agencies, independent agencies. Required forms may include: 

Disciplinary Records Request
Disclosure of Disciplinary Action
College Conduct Report
Dean's Certification Form

Step 1
Complete the student portion of the form, including the release authorization and date. Please make sure all required areas under the student section are completed. We cannot complete the form without your signature. 

Step 2
After you have completed the required student section, email it to deansclearance@chapman.edu.

In the email, please attach or include the following: 

Clearance Form, completed as shown above
Full Name & Student ID Number

Step 3
We will send the form directly to the institution. You will receive a confirmation email when it has been sent.

Request Disciplinary Records

Step 1
Download the Student Conduct Request Form and fill it out completely. 

Step 2
Email the form to deansclearance@chapman.edu from your Chapman email. 

IMPORTANT: Disciplinary Records are protected under the Family Educational Rights and Privacy Act(FERPA) and will not be disclosed over the phone, as we are unable to verify the caller's identity.

FAQ

How long does it take to process a request? 
It can take up to 7 business days to process a request. Dean's Clearance Forms will be emailed directly to the institution or agency. We recommend submitting your request as soon as possible. 

Do I need to meet with the Dean of Students or any other University administrator to have my Dean's Clearance form processed? 
No, there is no need to meet with the Dean or another University administrator. Follow the instructions above to submit your request via email. Our office will reach out to you directly if more information is required. 

Is there a processing fee for the Dean's Clearance form? 
No, there is no processing fee associated with Dean's Clearance requests. 

If no form is provided by the school/institution to which I am applying, but a disciplinary clearance is required, what should I do? 
If no form is provided to you by the institution to which you are applying, the Office of Student Conduct may be able to provide a clearance letter on Chapman letterhead to be sent directly to the institution.
