Below is a list of all financial aid related forms and documents. Some are required as part of your initial application for aid. Others are offered on a case-by-case basis. As a general rule, only complete a form if you have been asked to do so. Most requests will be listed on your Tasks tile on your Student Center, while others may be directly emailed to you for completion.
Federal Student Loan Documents
Students who have been offered federal student loans must complete some online documents before aid can be disbursed. Documents are completed
online and confirmation will be sent directly to the school upon completion. Forms
typically only need to be completed one time and are applied to subsequent years of
your program.
Loan Entrance Counseling ensures you understand the terms of your loans, as well as
your rights and responsibilities. The Master Promissory Note (MPN) is a legal promise
to pay your loan(s). Please note that there are separate MPNs for the Unsubsidized
Direct Loan and Direct Graduate PLUS Loan.
Verification Documents
The Department of Education selected a portion of applicants for secondary verification. If selected you will find tasks in your Tasks tile on your Student Center. Students selected for identity verification must visit our office in person to present your ID. If you cannot come in person you can request a virtual appointment. Students selected for income verification can complete the Independent Verification Worksheet and email it to gradfinaid@chapman.edu.
Budget Increase Request
Your cost of attendance also includes indirect costs like housing and food, books, transportation, etc. These items are estimates based on student survey data.
Cost of Attendance standards are based on federal regulations. If you review your aid package and believe you have essential costs that are not included, or have extenuating circumstances, you can complete a Budget Increase Request to ask for assistance. You may be asked to provide documentation to support your situation.
Satisfactory Academic Progress Appeal
In order to receive federal student aid, you must meet Satsfactory Academic Progress (SAP) standards. Students who fail those standards may submit an appeal to be reviewed by the committee. You will be sent a letter by email with the details of what standard was not met. The appeal and supporting documents can be submitted by email to gradfinaid@chapman.edu
TEACH Grant Request
The Federal TEACH (Teacher Education Assistance for College and Higher Education) Grant provides funding to students in an eligible program of study who agree to teach full
time for at least four years in a high-need field at a low income school or educational
service agency. Students who have reviewed and completed all requirements can submit
the request form by email to gradfinaid@chapman.edu. The deadline for Spring 2026 is November 1, 2005.
Release Authorization
The Family Educational Rights and Privacy Act (FERPA) affords student certain rights with respect to their education records, including
the right to inspect their education records, request an amendment of the records
that the student believes are inaccurate, and the right to control disclosures of
their records except to the extent that FERPA authorizes disclosure without consent.
Students may provide consent for Chapman University to share information with specified
parties by completing a FERPA Chapman Release Authorization eForm.
