Chapman University Student Health (CUSH), is staffed with professionally licensed nurses, nurse practitioners and physicians. We feel your health plays a vital role in how successfully you achieve your academic, social and personal goals during your college years. Good health promotes favorable results, poor health can be a major liability.

+-Services Offered

Our health care services usually include the following:

First aid: Cuts, abrasions, and burns.  Acute illness: Fever, sore throat, sudden pain, vomiting, and diarrhea.  Doctor's hours: Physician and Gyn Nurse Practitioner visits are available by appointment.  Health maintenance: Immunizations, Hepatitis B vaccine, TB skin testing, and tetanus boosters.Health screening: Pregnancy, PAP, STDs, blood pressure, and diabetes screening.

If the medical condition is beyond our scope of service, an outside referral is made. We have contracted physicians in the community who will accept our Chapman Student Health Insurance upon referral. Specific services (lab, x-ray, ultrasound, mammogram) may be overed with a Student Health Referral.

+-Am I Eligible for Services?

All students are required to submit a healthreview and immunization form.

Undergraduate Students

Services are available to all full-timeundergraduate students, as well as to any other students who have paid the Student Health Services fee. Students must have medical insurance and may choose to retain the basic indemnity plan provided by Chapman.

Services include, but are not limited to, first aid, treatment of acute illnesses, health maintenance and health screening and are provided by physicians, nurses, and nurse practitioners in our convenient,on-campus site.

Graduate Students

PLEASE NOTE: YOU ARE NOT AUTOMATICALLY ENROLLED FOR HEALTH SERVICES ON CAMPUS, OR IN THE CHAPMAN MEDICAL PLAN.

If you choose to use Chapman Health Services, you must request that the Health Services fee be applied to your account and submit a completed Health Record and Immunization forms.  By requesting Chapman Health Services, you are obliged to have health insurance.You may choose to have Chapman's medical insurance OR if you have your own plan, you may waive the Chapman plan by completing a Health Insurance Waiver form.

Location and Hours

The Chapman Student Health Center is located at the corner of Sycamore and Glassell.

The Health Center is open Monday through Friday 8:30am to 4:30pm. Walk-In hours are 8:30am-12:30pm during regular semesters.

An appointment is usually needed to see the doctor or gynecological nurse practitioners. Please call to schedule an appointment with a health care provider. Appointments are needed in the afternoons. Doctors' hours vary according to their availability.

Messages left on voice mail at the Health Center will be answered as soon as possible. Please remember to leave your phone number with your message.

This "walk-in" clinic is open from 8 am to 10 pm and does not usually require an appointment. In order for the insurance to pay your charges, you will need to report your visit to the Health Center promptly. Students who have waived the medical insurance should contact their own insurance carrier for emergency referral. The Sunrise clinic accepts many types of medical insurance.

Jacqueline Deats Jacqueline Deats, R.N.P, Director. Also a Gynecological Nurse Practitioner, Jaqueline is associated with Dr. Linzey in Orange.

Julie Mullican Julie Payn, Office Manager. Julie is a medical assistant, medical insurance claims manager and came to Chapman University Health Services in 1991.
Judy Taylor Judy Taylor, B.S.R.N., Staff Nurse. After working for 15 years at Fullerton College in Student Health, Judy came to Chapman Health Services in 2005.  Judy received her Bachelors of Science in Nursing from Arizona State University.
Thomas Blair Thomas Blair, MD. Dr. Blair is board certified in internal medicine and is in private practice in Orange.

Health Insurance Waiver

The deadline for turning in your Spring term Insurance waiver was 2/18/2012
