Requirements to Become a Preceptor


  • Affiliation Agreement with your Site/Organization is in place.
  • Complete the online preceptor application which includes:
    • CV
    • License Number
    • Preceptor history
  • Complete a Preceptor Availability worksheet
  • Complete the Chapman University background check
