Pre-Enrollment Requirements

As part of the enrollment process at Chapman University students are required to complete a Health Record and Immunization form. Please return the completed form to Student Health Services at:

Failure to submit form may delay your ability to register for classes.

If you are planning to participate in a team sport you are required to submit an Athletics Physical Exam Form.  Pam Gibbons, Head Trainer of the Athletic Department will provide the form.
