Insurance and Insurance Waivers
While all full-time undergraduates may use Health Services, all Chapman undergraduate students are also required to have health care insurance. Chapman University offers a student health insurance plan for which full-time undergraduates are automatically charged. Although not a fully paid comprehensive medical plan, it is designed to give financial assistance to students should the usual, simple sicknesses and/or accidents befall them. The medical benefit also covers some diagnostic procedures performed at the Health Center (urine and blood tests, etc.). Chapman Student Health Insurance is available to part-time and graduate students upon request at time of enrollment. Plan I is automatically charged to an incoming undergraduate. Plan II is available upon request.
The Chapman insurance policy, either Plan I or Plan II, is designed to meet your basic health needs. Chapman Student Health Insurance is underwritten by United Health Care. Please visit their website at www.studentresources.net for brochures, claims status, and other services. Enrollees are covered by the Chapman insurance while on or off campus, August through August. Alternative Coverage - If you do not meet the eligibility requirements or require optional coverage or dependant coverage, medical insurance plans are available to you. Contact Student Health Services to obtain additional information, brochure and application. These plans are not administrated through Chapman Student Health Services. Health Insurance WaiversAlthough we highly recommend retaining the Chapman Student Health Insurance, you may choose to waive the insurance premium fee. If you choose to waive the health insurance, please fill out completely the Application for Health Insurance Waiver form. The form is now available online Student Health Insurance waiver. The credited amount will be reflected in your next billing statement. Proof of medical insurance and a local medical provider or HMO is required to waive the Chapman medical insurance. Chapman Student Health is not considered a provider of services for private health care plans; therefore, we cannot bill your insurance company. An application for each academic year (August through May) must be completed before the posted deadline date. Students entering in the Spring semester must waive at the time of registration and again at the beginning of each academic year. Remember - You are not waiving your ability to use the student health services benefit, you are waiving the Chapman Student Health INSURANCE COVERAGE. |
