Student Health Services

Physical Therapy students are required to pay the Health Service fee and are also required to have health insurance, due to the nature of the program.  Students are automatically enrolled in the Chapman Student Health Insurance Plan, but students may elect to waive the insurance plan.  Proof of medical insurance and a local medical provider or HMO is required to waive the Chapman medical insurance. Chapman Student Health is not considered a provider of services for private health care plans; therefore, we cannot bill your insurance company. An application for each academic year (August through May) must be completed before the posted deadline date. Students entering in the Spring semester must waive at the time of registration and again at the beginning of each academic year.

Students are required to have a physical examination and history on file in the event that you need help from the Student Health Center. As a part of the physical examination, your physician will indicate your ability or lack of ability to perform the Technical Standards and will indicate any accommodations which might be required. Various immunizations are also required for both Student Health and for the Department of Physical Therapy. Copies of your medical records must be submitted to the Department of Physical Therapy prior to the start of the program. Students are also expected to maintain and provide copies of immunization records to the Department of Physical Therapy throughout the duration of the program.
