For 2012, all the insurance carriers will remain the same. We are especially pleased that most of the premium rates for our health and welfare plans will not increase. See the details about premiums, as well as a few changes outlined below. · Contributions: Employees participating in the Anthem PPO, Lumenos PPO+HSA, Kaiser Permanente, Delta Dental PPO, DeltaCare USA, Vision Service Plan and the Hyatt Legal Plan will be happy to know that there will be no increase in premiums for these plans. Beginning in January 2012, premium rates for the Anthem HMO will increase by 12%. Premium increases are equally shared by Chapman University and participating employees. · Anthem Prescription Program: Prescription drugs covered under the Anthem PPO and HMO plans will be categorized into a 4-Tier Prescription Program instead of the current 3-Tier Program. Tier 1 are the lowest cost drugs, mostly generic at a $10 copay. Tier 2 are more affordable brand name drugs at a $20 copay. Tier 3 are higher cost brand-name drugs at a $35 copay. Tier 4 are specialty drugs used to treat complex chronic conditions (typically self-injectible) with varying copays based on the cost of the drug up to a maximum of $150 per fill, and must be ordered throught the Specialty Pharmacy Program. · Dependent Eligibility Expanded: You may now enroll your dependent children up to age 26 regardless of student status on any of the medical, dental or vision plans! · Vision Enhancements: The VSP Buy-Up vision plan now allows contact wearers to get frames in 12 months, rather than 24 months. · Staff Short Term Disability: Short Term Disability Insurance (STDI) for staff through The Standard will continue to be voluntary (paid 100% by the employee). Employees selecting STDI will pay premiums on a post-tax basis. This means that individuals who file claims and receive weekly payments as shown under Coverage Options from STDI, will no longer pay taxes on those benefits. The Short Term Disability Plan elimination period has also been enhanced to match the 7-day elimination period of the California State Disability Insurance (CASDI) Plan. This is an improvement from the current year elimination period of 14 days. · Long Term Disability: Our Long Term Disability plan will still be paid by the University; however, benefits for those who file claims will now be tax-free. This is possible because the premium of this benefit will be taxable as imputed income on your paycheck.

To designate your consent to receive the electronic W2 form, you will need to log-in to Self-Service and click the "Pay Information" link in the menu. Then select "Annual Statement Consent Form". Follow the instructions to process your request.
