Registration

To register for any of the on-campus Leadership Services Program offerings please follow the instructions below based on the location of your program or seminar.

Irvine Campus

Our Irvine campus is located at 7545 Irvine Center Drive, Suite 150, Irvine, CA 92618. View campus map.  To register for this campus:

• Download and complete this Registration Form (PDF). 
• Fax the completed registration form to Ezra Huang at 714-997-6641 (f).
• For any questions please contact Ezra via email at ehuang@chapman.edu. Advance registration and payment are required.
• Registration Confirmation:  You will receive a confirmation within one week of your registration. Please contact him if you do not receive confirmation of your registration by either letter or email. 
• Multiple registrations:  Organizations have the option of completing a separate registration form for each employee or submitting one registration form with an attached document indicating name, phone number, email address and physical address (if different from company address) for each participant enrolling in the program.  
• Program Information:  One week prior to your program start date you will receive complete program information including parking instructions, course schedule, etc.
• Program Discounts:  Organizations enrolling 5 or more employees in the same certificate program receive a 10% discount on the registration cost for each participant.  The discount must be indicated on the registration form to qualify.
• Refund Policy: For programs with only one or two meetings, students must request a refund before first class. For programs with three or more meetings, students must request a refund before the second class. All refund requests must be made in writing and received before the deadline (an e-mail is acceptable). A $20 processing charge will be withheld from all refunds. The cost of materials provided at the start of the course may not be refundable. Students may request a deferred enrollment for the full amount of the course if they plan to enroll in the same course within one year. The processing fee will be waived for a deferred enrollment.
