Self-Study Report and Appendix Overview

The Program Review Self-Study Report is based on the questions provided in the Program Review Guidelines and focused on central issues and questions determined by the department/school in consultation with the Dean and the Office of the Chancellor.  The following documents should be used in the program review report to conduct a thoughtful and productive review: Department/School Documents and Information for Program Review, Institutional Data for Program Review, and University Documents for Program Review.

General Guidelines
1. The Program Review Self-Study Report should be 10-12 pages and include each of the following five sections: 1) Program Description; 2) Educational Goals and Effectiveness; 3) Faculty Evaluation and Development; 4) Internal Review, External Measures, and Accreditation; and 5) Future Goals and Actions.  Please see Program Review Self-Study Report Format and Guidelines for page length requirements, program review questions, and applicable resources and requirements.

a.    All academic units are expected to address the questions in each section of the Program Review Guidelines.  Individual units, in consultation with the Dean and the Office of the Chancellor, may determine which questions in each section are most relevant to their primary goals and focus on those questions.  If the individual unit determines that some questions are less applicable or inapplicable to their program review, the unit may state so in its program review.  The unit may also add questions, in consultation with the Dean and Office of the Chancellor, that help reflect its strategic goals and activities.

b.   The Program Review Self-Study Report should provide an Appendix which includes: 1) Department/school documents and information provided by the school/department (see Department/School Documents and Information for Program Review); 2) Institutional data provided by the Office of the Chancellor (see Institutional Data for Program Review); and 3) Other department/school documents/information.

c.   The Program Review Self-Study Report format should include: 1) size 12 Times New Roman type; 2) one-inch margins; and 3) all pages should be single-sided, hole-punched, and placed in the six binders provided by the Office of the Chancellor.  The academic unit should keep two copies (one for the unit and one for the Dean) and submit four copies to the Office of the Chancellor (two copies for the external reviewers, one for the Academic Council Assessment Committee, and one for the Office of the Chancellor).

2. The program review must include both undergraduate and graduate level programs offered by the department/school. The academic unit’s program review should examine all degree programs on the undergraduate and graduate levels in the department/school with the exception of any programs that the academic unit has received approval from the Office of the Chancellor not to examine.

3. Academic units may be asked by the Dean or the Office of the Chancellor to address specific questions and issues based on review of department/school documents and information, institutional data on the department/school and its programs, and university documents, and to revise their courses/curriculum as necessary, following relevant curriculum guidelines.  Examples of specific issues may include: 1) frequency of course offerings and number of enrollments for specified courses; 2) compliance of department majors, minors, and emphases with existing curriculum guidelines; and 3) number and percentage of cross-listed undergraduate courses in graduate programs.

1. Program Review Self-Study Report and Appendix Format and Guidelines – Provides an overview of the Program Review Self-Study Report format, questions, and required documents, data, and information.

2. Department/School Information and Documents for Program Review – Lists documents and data that departments/schools should use throughout the Program Review Self-Study Report and must provide in the Appendix.

3. Institutional Data for Program Review – Lists subjects on which institutional data is available, and appropriate sections and uses for data in the Program Review Self-Study Report and Appendix.

4. University Documents for Program Review – Lists university documents that departments/schools are expected to utilize, links to where such documents are available online, and sections of the Program Review Self-Study Report where such documents are applicable.
