Forms for Student Internships

  • Internship Learning Agreement Form (ILAF): This is the contract between you, your faculty advisor, and your employer. Obtain paper copy at the CDC in Argyros Forum. The form registers you for your internship and consists of four parts:
    1. Student Information:
      • Provide your complete name, local mailing address, phone number, email address, and student ID number
      • the faculty internship advisor is the faculty member who will supervise your internship, and the department name corresponds to where that faculty member is located
      • Indicate the number of units you will be taking and the term for which you are enrolling
    2. Faculty Internship Advisor:
      • Work with your faculty internship advisor to complete Section B and identify what academic objectives will occur from the internship experience
      • Have your advisor identify two or three methods of evaluation which will best reflect and illustrate your learning from the internship
    3. Employer Information:
      • Have your employer complete Section C with his/her name, title, and mailing address including the company, street address and suite number, or other identification. We must have the employer’s phone number and the name of your direct supervisor
      • Be sure the employer offers a detailed description of your internship duties or your ILAF will not be approved.
      • If employers are unclear about what comprises a detailed job description, they may contact the Internship Office (ccintern@chapman.edu) for assistance.
    4. Signatures:
      • Without the signatures of you, your faculty internship advisor, and your internship employer, the learning agreement is incomplete and invalid. Signatures from all parties are required before the ILAF will be approved and your registration completed.
  • Internship Responsibilities Form (IRF): This form outlines your accountibility to your internship employer and to the internship program in general. This would include such things as completing the hours agreed upon within the term of your internship, showing up to work on time, submitting the forms on time, etc. Obtain paper copy at the CDC in Argyros Forum.
